目的 调查分析云南省20 012名妇女人乳头状瘤病毒(HPV)3年内的新发感染状况,分析HPV型别和人群分布特征。 方法 在云南省4个HPV检测项目监测县,为35~64岁,已婚/有性生活且近三年未接受过宫颈癌筛查或HPV检测的妇女提供宫颈癌筛查,收集其一般人口学特征,同时采集其采集阴道分泌物和宫颈黏膜组织标本,使用基因扩增技术及导流杂交原理,对采集的标本进行 23 种HPV 基因型进行分型检测,使用检验方法分析云南省农村妇女HPV感染现状及人群分布。结果 20 012名农村妇女HPV3年内新发感染率为7.03%,单一高危亚型感染5.46%,单一低危亚型感染率为0.14%;混合感染率为1.43%,混合感染中均感染至少一种高危亚型。HPV 亚型感染检出率前5位依次是HPV52、HPV16、HPV58、HPV51和HPV18。云南省A、B、C、D4个地区农村妇女的HPV感染率分别为6.86%、8.34%、7.12%、5.80%,差异有统计学意义(χ2 = 24.97,P<0.01);四个地区HPV高危亚型感染率分别为6.52%、8.30%、7.10%、5.64%,差异有统计学意义(χ2 = 29.06,P<0.01);35~44岁、45~54岁和55~64岁三个年龄组的HPV感染率分别为6.77%、6.34%、9.00%,差异有统计学意义(χ2 =31.19,P<0.01);三个年龄组高危亚型感染率分别为6.58%,6.25%,8.85%,差异有统计学意义(χ2= 31.01,P<0.01)。 结论 HPV感染妇女以高危型别单一感染为主,HPV感染的主要基因型为HPV52和HPV16型。不同地区和不同年龄组间农村妇女的HPV感染率不同。
Abstract:
Objective To investigate and analyze the status of new human papillomavirus (HPV) infections in 20 012 women in Yunnan Province over a 3-year period, and to analyze the characteristics of HPV type and population distribution. Methods In four HPV testing program surveillance counties in Yunnan Province, women aged 35 to 64 years who were married/sexually active and had not received cervical cancer screening or HPV testing in the past three years were offered cervical cancer screening, and their general demographic characteristics were collected. Meanwhile the vaginal secretions and cervical mucosal tissue specimens were collected and tested for 23 HPV genotypes using gene amplification technology and the principle of infusion hybridization. The current status and population distribution of HPV infection in rural women in Yunnan Province were investigated using the chi-square test. Results The rate of new HPV infections in 20 012 rural women within 3 years was 7.03%, 5.46% for single high-risk subtypes, 0.14% for single low-risk subtypes, and 1.43% for mixed infections, all of which were infected with at least one high-risk subtype. The prevalence of HPV infection among rural women in Yunnan Province was 6.86%, 8.34%, 7.12%, and 5.80% in regions A, B, C, and D, respectively, with statistically significant differences (χ2=24.97, P<0.01). The prevalence of HPV high-risk subtypes in the four regions was 6.52%, 8.30%, 7.10%, and 5.64%, respectively, with statistically significant differences (χ2=29.06, P<0.01). HPV infection rates in the three age groups of 35 to 44, 45 to 54, and 55 to 64 years were 6.77%, 6.34%, and 9.00%, respectively, with statistically significant differences (χ2=31.19, P<0.01). High-risk subtype infection rates in the three age groups were 6.58%, 6.25%, and 8.85%, respectively, with statistically significant differences (χ2=31.01, P<0.01). Conclusion HPV-infected women are predominantly infected with a single high-risk type, and the main genotypes of HPV infection are HPV52 and HPV16. The prevalence of HPV infection varied among rural women in different regions and among different age groups.
