目的 描述与分析1990—2019年中国胰腺癌疾病负担状况及其变化趋势,为有效开展胰腺癌的防控工作提供科学依据。方法 利用2019年全球疾病负担研究(Global Burden Health,GBD)结果,使用Excel 2019,按不同性别、年龄组对1990—2019年中国胰腺癌的发病率、死亡率和DALY率情况进行描述,使用Joinpoint软件计算其年度变化百分比值与平均年度变化百分比值。结果 与1990年相比,中国胰腺癌发病率由2.26/10万上升至8.08/10万,死亡率由2.29/10万上升至8.25/10万,DALY率由63.31/10万增加至197.22/10万,均呈不同幅度的上升趋势(P<0.05)。中国人群胰腺癌疾病负担男性较女性重,标化发病率、死亡率、DALY率的AAPC值均为男性(2.20%、2.10%、2.00%)大于女性(1.90%、1.80%、1.50%)。随年龄的增长各率均有先上升后下降的趋势,发病与死亡率峰值在85~90岁年龄段,DALY率峰值在70~74岁年龄段,各年龄段男性各率均高于女性。结论 1990—2019年中国人群胰腺癌疾病负担不断加重,老年、男性为高危人群,需有针对性的做好胰腺癌的防治工作。
Abstract:
Objective To describe and analyze the disease burden and trend of pancreatic cancer in China from 1990 to 2019, and to provide scientific basis for effective prevention and control of pancreatic cancer. Methods Using the results of the Global Burden Health(GBD)2019 study and Excel 2019, the incidence, mortality and DALY rates of pancreatic cancer in China from 1990 to 2019 were described by gender and age group. Joinpoint software was used to calculate the annual percentage change and the average annual percentage change. Results Compared with 1990, the incidence rate of pancreatic cancer in China increased from 2.26/100 000 to 8.08/100 000, mortality rate increased from 2.29/100 000 to 8.25/100 000, and DALY rate increased from 63.31/100 000 to 197.22/100 000, all with different magnitudes of increase(P<0.05). The disease burden of pancreatic cancer in the Chinese population was heavier in men than that in women, and the AAPC values of standardized incidence, mortality, and DALY rates were greater in men(2.20%, 2.10%, 2.00%)than those in women(1.90%, 1.80%, 1.50%). There was a tendency for each rate to increase and then decrease with age, with peak morbidity and mortality rates in the 85-90 age group and peak DALY rates in the 70-74 age group, and each rate was higher in males than in females in all age groups. Conclusion From 1990 to 2019, the burden of pancreatic cancer has been increasing in Chinese population, and the elderly and males are high-risk groups, which require targeted prevention and treatment of pancreatic cancer.
