摘要:
目的 分析户籍与流动孕产妇妇幼保健健康教育现状，探索不同特征对孕产妇妇幼保健健康教育的影响，为提升孕产妇妇幼保健水平提供针对性的政策建议。 方法 利用2017年全国流动人口卫生计生动态监测调查中国内8城市户籍与流动人口对比专题数据，该调查采用多阶段分层PPS抽样原则进行调查。采用描述性分析，χ2检验，logistic回归分析对国内8城市的户籍与流动孕产妇的妇幼保健健康教育情况进行分析。 结果 本研究纳入孕产妇共计820名，户籍和流动孕产妇的妇幼保健健康教育接受率差异较大，分别为83.26%和60.66%，接受方式以宣传资料（纸质、影视）和宣传栏/电子显示屏为主；其中有无子女（OR = 0.382, 95%CI：0.207～0.704, P＜0.01），国家基本公共卫生服务知晓情况（OR = 0.198, 95%CI：0.105～0.374, P＜0.01）以及健康档案建立情况（OR = 0.237, 95%CI：0.105～0.531, P＜0.01）是流动孕产妇接受妇幼保健健康教育的影响因素；国家基本公共卫生服务知晓情况（OR = 0.156, 95%CI：0.085～0.287, P＜0.01）和健康档案建立情况（OR = 0.260, 95%CI：0.143～0.474, P＜0.01）是户籍孕产妇接受妇幼保健健康教育的影响因素。 结论 政府与基层卫生服务部门应努力破除制度壁垒，促进公共卫生资源利用最大化，以提高户籍与流动孕产妇整体妇幼保健健康水平。
Abstract:
Objective To analyze the current situation of health education for maternal and child health care in registered residence and floating pregnant women and explore the influence of different characteristics on health education of maternal and child health care for pregnant women, to provide targeted policy recommendations for improving maternal and child health care level. Methods The relevant data of comparison between the registered residence and the floating population in eight cities in China were retrieved from the national floating population health and family planning dynamic monitoring survey in 2017. The survey adopted the principle of multi-stage stratified PPS sampling. Descriptive analysis, chi-square test, and logistic regression analysis were conducted to analyze the health education of eight city and maternal and child health care in registered residence in China. Results A total of 820 pregnant women were included in this study. The acceptance rate of maternal and child health education of registered and floating pregnant women was significantly different, 83.26% and 60.66%, respectively. The reception methods were mainly publicity materials (paper material, film, and television) and bulletin boards/electronic display screens. Among them, whether they had children OR not (OR=0.382, 95%CI: 0.207-0.704, P<0.01), the awareness of national basic public health services (OR=0.198, 95%CI: 0.105-0.374, P<0.01), and the establishment of health records (OR=0.237, 95%CI: 0.105-0.531, P<0.01) were the influencing factors for floating pregnant women to receive maternal and child health education. The awareness of national basic public health services (OR=0.156, 95%CI: 0.085-0.287, P<0.01) and the establishment of health records (OR=0.260, 95%CI: 0.143-0.474, P<0.01) were the influencing factors of maternal and child health education for pregnant women with registered residence. Conclusion The government and grassroots health service departments should strive to break down institutional barriers and maximize the utilization of public health resources, so as to improve the overall maternal and child health level of registered and floating pregnant women.
