摘要:
目的 了解流动老人基本公共卫生服务利用情况，以及社会融合对基本公共卫生服务利用的影响。方法 于2021年7—12月，采用方便抽样的方法，抽取广州市、深圳市、江门市452名流动老人进行问卷调查。采用二元logistic回归模型分析流动老人社会融合对建立健康档案、接受健康教育和老年人健康管理之间的影响。结果 在452名流动老人中，60.8%建立了居民健康档案，接受过健康教育和老年人健康管理的分别有64.4%和60.0%。Logistic分析结果显示，流动老人的居民健康档案行为受到月消费水平（B = 0.368,P＜0.05）、社区文体活动参与程度（B = 0.362,P＜0.01）、居留意愿（B = 0.408,P＜0.001）、年龄（B = - 0. 513,P＜0.01）、户籍类型（B = 0.742,P＜0.01）、流动类型（B = - 0.539,P＜0.05）等变量的影响；健康教育服务利用行为受到社区文体活动参与程度（B = 0.354,P＜0.01）、年龄（B = - 0.411,P＜0.05）、户籍类型（B = 0.561,P＜0.05）的影响；老年人健康管理服务利用行为受到社区文体活动参与程度（B = 0.291,P＜0.01）、居留意愿（B = 0.239,P＜0.05）、年龄（B = - 0.474,P＜0.05）、流动类型（B = - 0.471,P＜0.05）、流动时间（B = 0.225,P＜0.05）、流动目的（B = 0.328,P＜0.01）的影响。结论 流动老人基本公共卫生服务利用率较低，社会融合会对流动老人的基本公共卫生服务利用产生积极影响。可以通过提高流动老人获得基本公共卫生服务的能力和社会融合水平来促进流动老人的基本公共卫生服务利用，从而实现基本公共服务均等化。
Abstract:
Objective To understand the utilization of basic public health services by the floating elderly and the impact of social integration on the utilization of basic public health services. Methods From July to December 2021, we used a convenience sampling method to recruit 452 floating elderly in Guangzhou, Shenzhen, and Jiangmen for a questionnaire survey. The binary logistic regression was used to analyze the impact of social integration of older migrants on the establishment of health records, health education, and elderly health management. Results Among the 452 older migrants, 60.8% established health records, and 64.4% and 60.0% received health education and elderly health management, respectively. The results of binary logistic regression showed that the establishment of health records was affected by the monthly consumption level (B=0.368, P<0.05), participation in community activities (B=0.362, P<0.01), residence intention (B=0.408, P<0.001), age (B=-0.513, P<0.01), household registration (B=0.742, P<0.01), and mobility type (B=-0.539, P<0.05). Health education service utilization behavior was affected by the participation in community activities (B=0.354, P<0.01), age (B=-0.411, P<0.05), and household registration (B=0.561, P<0.05). The elderly health management service utilization behavior was affected by the participation in community activities (B=0.291, P<0.01), residence intention (B=0.239, P<0.05), age (B=-0.474, P<0.05), type of migration (B=-0.471, P<0.05), time of migration (B=0.225, P<0.05), and migration reason (B=0.328, P<0.01). Conclusion The utilization level of the basic public health services by older migrants is generally low. Social integration has a positive impact on the utilization of basic public health services for older migrants. We can promote the utilization of basic public health services for the older migrants by improving their ability to obtain basic public health services and the level of social integration, to realize the equalization of basic public services.
